Chelsea vs. Tottenham 11/12 CL access

In a case in which the title holder doesn't qualify to CL by league position (like it happened in 11/12), what are the rules concerning this case - is the title holder always has a priority by UEFA rulings or is it up to the local association to decide who qualifies?

In 2005, when Liverpool won, it was up to the National FA to decide, although they were supposed to choose the CL winner. But the English FA chose Everton (4th placed), so UEFA gave Liverpool a wildcard. Since then, UEFA corrected the regulations and now the 4th placed team will be automatically transferred to EL (of course, only for countries that have 4 CL spots and the CL winner comes from such an association and finished below 4th).
